Meet our Case manager – Alan Dsouza.

With experience in aged care Alan uses his clinical knowledge to understand his client’s health conditions, and work with them to create a care plan that reaches their goals. He connects clients and their families with community services, allied health professionals and support workers, helping people access the right care for their needs.

Alan is passionate about helping seniors enjoy life to the full. He loves supporting people to live safely and independently at home – if they wish – for as long as possible and wants to see older people in his community thrive.

Meet our Case manager – Robert Broome.

Robert, with a background in psychology, is deeply committed to ensuring continuity of care and prioritizing the needs of each individual. His passion lies in collaborating with clients, their families, and fellow healthcare professionals. Drawing on his psychological expertise, Robert finds immense joy in helping others and believes that the greatest reward is seeing someone improve their quality of life.

Meet our Case manager – Shivani.

Shivani is a dedicated Nurse Case Manager with 5 years of experience in community care across Australia. She specializes in supporting seniors, individuals with disabilities, and those with chronic diseases. Shivani prioritizes building trusting relationships with patients and their families, ensuring open communication and collaboration.

 She is committed to professional growth and stays updated with the latest community care practices. Outside of work, Shivani enjoys yoga, dancing, and coffee, which help her recharge.

 We are confident that Shivani’s expertise and dedication will continue to improve healthcare outcomes in our community.
