Buelah Adagal- Director  – Dialysis Therapist

Home Caring Altona is led by healthcare professional Buelah Adagal, whose career has been dedicated to supporting people with complex and ongoing care needs. As a dialysis therapist, Buelah has worked closely with individuals living with chronic conditions, helping them manage their health while maintaining the best possible quality of life.

She brings a deep understanding of Australia’s aged care system, developed through six years of experience as an aged care assessor. This background gives her valuable insight into the challenges people and families face when navigating care services. Today, she uses this expertise to guide clients through both the aged care and NDIS systems, helping them access the right supports to make a meaningful difference in their daily lives.

Buelah is passionate about building genuine, trusting relationships with the people she supports. She takes a personalised approach to care, working closely with clients to understand their goals and preferences. Her focus is on empowering individuals to live as independently as possible, while ensuring they feel supported, respected, and confident in their care journey.

Alan Dsouza- Operations Manager

Alan Dsouza is an experienced aged care professional who has progressed into the role of Operations Manager after spending his first two years as a dedicated Case Manager. This hands-on experience has given him a strong clinical foundation and a deep understanding of client needs, which continues to inform his leadership approach.

Alan has taken on responsibility for overseeing daily operations, ensuring high standards of care, compliance, and efficient service delivery across the organisation. He is focused on supporting his team, strengthening processes, and driving continuous improvement to achieve the best outcomes for clients.

While leading operations, Alan also remains actively involved in case management, working closely with clients and their families to develop personalised care plans and connect them with the right supports. He is committed to delivering compassionate, person-centred care and supporting his community to thrive.

Khamoshi Shah – Case Manager

I am a dedicated and compassionate Case Manager, specialising in helping individuals navigate complex service systems and access the support they need to live more independent and fulfilling lives. My approach is person-centred, ensuring each client receives tailored support based on their unique needs and goals.

I work closely with clients, families, and support networks to conduct assessments, develop practical care plans, and connect people with essential services such as healthcare, housing, and community resources.

With strong problem-solving skills and an empathetic communication style, I am passionate about advocacy and empowering individuals to build confidence and overcome challenges with dignity.

As a proud mum, I bring empathy, resilience, and a genuine understanding of the importance of strong support systems into everything I do.
